hsCRP vs Standard CRP: What Is the Difference?

The two tests sound different but measure the very same protein. The real difference is precision, and it determines what each test can actually tell you.

The short answer

hsCRP (high-sensitivity CRP) and standard CRP measure the exact same protein: C-reactive protein. The difference is precision. The high-sensitivity assay can accurately detect very low concentrations, making it suited to measuring chronic low-grade inflammation for cardiovascular risk assessment. Standard CRP is calibrated for higher levels and is used to detect acute inflammation and infection, typically when values rise above roughly 5 to 10 mg/L.

If you have ever looked at lab options or a doctor's order and seen both CRP and hsCRP, you would be forgiven for assuming they measure two different things. In fact they measure identical molecules. C-reactive protein is a single substance made by the liver, and both tests report the same protein in the same units, milligrams per liter. What differs is the sensitivity of the laboratory method and, as a result, the questions each version is designed to answer.

That distinction matters more than it first appears. Choosing the wrong test for the question at hand can produce a result that is technically accurate but clinically unhelpful. Understanding what separates the two, and how each maps onto the interpretation of your CRP numbers, helps you make sense of why a wellness context calls for one and an emergency room calls for the other.

Same Protein, Different Sensitivity

One molecule, two assays. C-reactive protein is a ring-shaped protein released by the liver in response to inflammatory signals, chiefly the cytokine interleukin-6. It rises within hours of an inflammatory trigger and falls as the trigger resolves. There is only one CRP in your blood. The term hsCRP does not describe a special variant of the protein; it describes a more sensitive way of measuring the same molecule.

What high-sensitivity means. A standard CRP assay is designed and validated to measure the substantial elevations seen in active infection or major inflammation, where levels can climb into the tens or hundreds. It is less reliable at the low end of the scale. The high-sensitivity assay uses methods that accurately quantify CRP down to a fraction of a milligram per liter. That extra precision at the bottom of the range is the entire point, because chronic low-grade inflammation lives in exactly that low territory.

Why the low end matters. The kind of inflammation linked to long-term cardiovascular risk is subtle and persistent, often producing CRP values well below the threshold a standard assay was built to resolve. A standard test might simply report such a result as low or within normal limits, missing the fine gradations that carry meaning. The high-sensitivity assay can distinguish, for example, a value near 0.5 mg/L from one near 2.5 mg/L, and that difference is precisely what cardiovascular risk stratification depends on.

When Standard CRP Is the Right Test

Acute inflammation and infection. Standard CRP shines when the clinical question is whether significant acute inflammation is present. In infection, tissue injury, or a flare of an inflammatory condition, CRP can rise rapidly and substantially. Values above roughly 5 to 10 mg/L generally indicate meaningful acute inflammation, and much higher values can accompany serious bacterial infections. In these situations, the precision of the high-sensitivity assay is unnecessary because the signal is large.

Monitoring known conditions. Clinicians use standard CRP to track the activity of established inflammatory diseases and to follow the course of infections and recovery. Because the values of interest are high, the standard assay captures them well and is the appropriate, cost-effective choice. Here the goal is to watch a large, moving number fall as treatment takes effect, not to detect a whisper of chronic inflammation.

Why hsCRP is not used acutely. During an acute illness, both assays would read high, but the high-sensitivity number loses its special value. Its usefulness comes from resolving low concentrations, and when inflammation is roaring, that fine resolution adds nothing. This is also why an hsCRP measured during an infection cannot be interpreted as a cardiovascular risk marker; the acute signal overwhelms the chronic one.

When hsCRP Is the Right Test

Cardiovascular risk assessment. The signature use of hsCRP is estimating cardiovascular risk from chronic low-grade inflammation. Here the recognized bands are an hsCRP below 1 mg/L for low relative risk, 1 to 3 mg/L for average risk, and above 3 mg/L for high risk. These distinctions all sit in the low range that only a high-sensitivity assay can measure reliably, which is why the high-sensitivity version is the standard for this purpose. Tracking wellness and lifestyle change. Because it resolves small differences, hsCRP is well suited to monitoring how chronic inflammation shifts over time in response to diet, exercise, weight, and sleep. A person working to reduce their inflammatory baseline needs a test that can detect a move from, say, 2.8 to 1.6 mg/L, a change a standard assay would blur. Interpreting hsCRP correctly. One caveat: even the high-sensitivity assay reads the same protein, so a genuinely acute event such as a cold, injury, or dental infection will push an hsCRP up temporarily. For cardiovascular interpretation, the measurement should be taken when you are well, and a single very high value should prompt a repeat once any acute illness has resolved. The assay is sensitive, but it is not able to tell chronic from acute inflammation on its own; that judgment comes from context.

Choosing the Right Test for Your Goal

Match the test to the question. The practical rule is simple. If the question is whether you have a significant acute inflammation or infection, standard CRP answers it.
