The Paleo Diet and Inflammation: Does Eating Like an Ancestor Reduce CRP?

The paleo diet is built on a simple premise: eat the whole foods our ancestors ate and skip the modern processed ones. That premise removes some of the biggest inflammatory offenders, but it also cuts out foods with real anti-inflammatory value.

The short answer

The paleo diet may modestly lower inflammation for some people, mostly because it strips out ultra-processed foods, refined sugar, and refined grains, which are among the most reliable dietary drivers of higher CRP. A handful of small studies have shown reductions in inflammatory markers on paleo, though they are short and hard to separate from the weight loss that often accompanies the diet. The main drawback is that paleo also excludes legumes and whole grains, foods that supply their own anti-inflammatory fiber and are staples of the best-studied anti-inflammatory diets. The evidence is genuinely mixed.

The paleo diet, short for paleolithic, is based on the idea of eating the way humans did before agriculture: meat, fish, eggs, vegetables, fruits, nuts, and seeds, while avoiding grains, legumes, dairy, refined sugar, and processed foods. The underlying argument is that our bodies are better adapted to these ancestral foods than to the products of modern agriculture and food processing, and that returning to this pattern reduces the chronic inflammation behind many modern diseases.

Whether that argument holds up under scrutiny is more complicated than the marketing suggests. Some of what paleo removes is genuinely inflammatory, and cutting it out can help. But some of what paleo removes is beneficial, and excluding it comes at a cost. Understanding the net effect on inflammation means looking honestly at both sides of the ledger rather than accepting the ancestral framing at face value.

It is also worth noting up front that the evolutionary story behind paleo is more of a marketing narrative than a settled scientific account. Ancestral human diets varied enormously by geography and season, many groups did in fact eat wild grains and legumes, and modern paleo foods bear little resemblance to their wild counterparts. None of that makes the diet good or bad on its own; it simply means the case for paleo should rest on what it does to measurable outcomes like inflammatory markers, not on assumptions about how our ancestors ate. With that in mind, the rest of this article focuses on the food choices themselves and what the evidence actually shows about their effect on inflammation.

What Paleo Removes That Can Lower Inflammation

Cutting ultra-processed foods. The most defensible part of the paleo diet is that it eliminates ultra-processed foods, which are among the most consistent dietary contributors to higher inflammatory markers. Packaged snacks, sugary drinks, fast food, and industrially formulated products largely disappear on paleo. For someone whose baseline diet was heavy in these foods, this change alone could plausibly lower their CRP, and it is the single strongest reason the diet might reduce inflammation.

Removing refined sugar and refined grains. Paleo eliminates added sugar and refined grains like white bread and white rice. Diets high in refined carbohydrates and added sugar promote blood sugar spikes and oxidative stress that feed inflammatory signaling. By replacing these with vegetables, fruits, and whole-food protein sources, paleo tends to flatten blood sugar responses and reduce the inflammatory pressure that comes with a high glycemic load. This is a real and well-supported mechanism.

More whole foods overall. By design, paleo shifts eating toward whole, minimally processed foods and increases vegetable, fruit, nut, and seed intake for many people. That shift brings in more fiber, polyphenols, and micronutrients than a typical processed diet provides. In this respect, paleo shares meaningful common ground with a general anti-inflammatory diet, and much of its potential benefit comes from this whole-food emphasis rather than anything unique to the ancestral concept.

An emphasis on omega-3s and healthier fats. Paleo tends to favor fatty fish, nuts, and seeds while steering away from the refined seed oils and trans fats common in processed foods. Fatty fish supply the long-chain omega-3 fatty acids EPA and DHA, which play a documented role in resolving inflammation, and nuts and seeds add fiber alongside their fats. To the extent that a paleo dieter genuinely leans into seafood, olive oil, avocado, nuts, and seeds rather than defaulting to large portions of red meat, the fat profile of the diet can shift in a direction the evidence associates with lower inflammatory markers. This is another area where the benefit depends far more on the specific food choices than on the paleo label itself.

What Paleo Excludes That May Work Against It

The case for legumes. Paleo excludes all legumes, including beans, lentils, and chickpeas, on the grounds that they were not part of pre-agricultural diets. From an inflammation standpoint this is hard to justify, because legumes are among the richest sources of fiber and plant protein and are strongly associated with lower inflammatory markers and better metabolic health in nutrition research. Removing them sacrifices a category of food that the evidence generally supports as anti-inflammatory, in service of a historical rule rather than a biological one.

Whole grains carry benefits too. Paleo also excludes all grains, including whole grains like oats, barley, and brown rice. While refined grains are a fair target, whole grains are a different story: they provide substantial fiber and have been associated with lower inflammation and reduced disease risk in large observational studies. Lumping whole grains together with refined grains and cutting both is one of the clearest places where paleo diverges from the mainstream evidence on anti-inflammatory eating.

The fiber trade-off. Because it removes two major fiber-rich food groups, a poorly planned paleo diet can actually end up lower in fiber than expected, especially if it leans heavily on meat and light on vegetables. Since fiber feeds the gut bacteria that produce anti-inflammatory short-chain fatty acids, this is a meaningful trade-off. A paleo dieter can compensate by eating generous amounts of vegetables, fruits, nuts, and seeds, but doing so requires deliberate effort rather than following the diet's exclusions alone.

Cost, sustainability, and how it is practiced. Paleo can also skew toward large quantities of meat, which raises the practical questions of cost and long-term adherence, and which brings in the concerns tied to red and processed meat if those become the diet's centerpiece. A version of paleo built around abundant plants with modest portions of quality protein looks very different, from an inflammatory standpoint, than one built around heavy meat consumption. As with most dietary patterns, the label alone conceals a wide range of ways it can be executed, and two people both calling themselves paleo can end up with quite different inflammatory profiles depending on where they place the emphasis.

What the Studies Actually Show

Small trials, encouraging but limited. Several small, short-term studies have reported reductions in inflammatory markers, along with improvements in weight, blood pressure, and blood sugar, when people switch to a paleo diet. These findings are genuinely encouraging, but the studies tend to be small, brief, and lacking long-term follow-up, which limits how much weight they can carry. There are no large, long-term randomized trials establishing that paleo reduces inflammation better than other whole-food dietary patterns.

Weight loss as a confounder. Many paleo studies involve weight loss, and because excess fat tissue is itself a source of inflammatory signaling, losing weight lowers markers like CRP regardless of the specific diet used to achieve it. This makes it difficult to know how much of paleo's apparent anti-inflammatory effect comes from the food choices themselves versus the weight loss that often accompanies any structured whole-food diet. It is a recurring limitation across the diet research literature, and paleo is no exception.

Comparison with better-established diets. When paleo has been compared to other healthy dietary patterns such as the Mediterranean diet, results have been mixed, with paleo sometimes performing comparably in the short term but without the decades of population-level evidence that support Mediterranean-style eating. Given that the Mediterranean pattern includes the legumes and whole grains that paleo excludes, and has a far deeper evidence base, it remains the more strongly supported choice for reducing inflammation. Paleo can be a reasonable improvement over a heavily processed diet, but the ancestral framing is not necessary to explain its benefits, and its exclusions are not well justified by the evidence.
