Does Bromelain Reduce Inflammation?

Bromelain is the protein-digesting enzyme from pineapple, and it has genuine anti-inflammatory activity. But there is an important gap between the enzyme's reputation and what the best human trials actually tested. Here is the honest evidence, dose, and safety.

The short answer

Bromelain, a mix of enzymes extracted from pineapple, has real anti-inflammatory activity in the lab and in some human trials. Randomized studies show it can meaningfully lower C-reactive protein (CRP) after surgery: in one trial of hip replacement patients, an oral enzyme combination containing bromelain produced a 32 percent lower CRP curve than placebo. The catch is that most of the strongest human evidence tested bromelain combined with other enzymes, not bromelain alone, and at least one well-run trial found no effect at all. Bromelain is promising in specific situations, especially post-surgical recovery, but it is not a proven general inflammation cure.

Bromelain is a group of protein-digesting (proteolytic) enzymes concentrated in the stem and fruit of the pineapple plant. It has been used for decades in Europe as a supplement for swelling, bruising, and recovery, and it is one of the more genuinely interesting natural anti-inflammatory candidates because, unlike many plant compounds, it survives digestion well enough to reach the bloodstream in an active form. That is a real advantage over poorly absorbed polyphenols. But bromelain also illustrates a recurring lesson in supplement science: an enzyme can be genuinely active and still leave you with a murky picture, because the human trials that look most impressive rarely test the ingredient by itself.

In human randomized trials it has lowered CRP after surgery, but usually as part of an enzyme combination rather than as an isolated ingredient, and results are not uniformly positive.

What Is Bromelain, and Why Would It Affect Inflammation?

Bromelain is best known as the enzyme that makes fresh pineapple tingle on your tongue and stops gelatin from setting: it breaks down protein. That same protein-cutting activity is the basis for its anti-inflammatory reputation. In laboratory and animal work, bromelain appears to interfere with several steps of the inflammatory cascade. It can reduce the production of pro-inflammatory signaling molecules such as tumor necrosis factor alpha and interleukin-6, dampen activation of the master inflammatory switch NF-kappa-B, and modulate the behavior of immune cells and the clotting and swelling pathways that accompany tissue injury. In an animal model of intestinal inflammation, oral bromelain lowered a panel of oxidative stress and inflammatory markers, including CRP, and researchers described its protective effect as the most potent among the enzymes they compared.

This is why bromelain has been studied most heavily in the context of injury and surgery, where the goal is to speed the resolution of acute swelling and pain rather than to treat a chronic disease. The enzyme is not acting like a nutrient your body was missing; it is acting more like a mild, natural modulator of the tissue-repair response.

Does Bromelain Lower CRP in Human Trials?

The clearest human signal comes from surgery. In a 2023 randomized, double-blind, placebo-controlled trial, patients undergoing elective total hip replacement took an oral enzyme combination containing 90 mg of bromelain (alongside trypsin and the flavonoid rutoside) before and after their operation. Compared with placebo, the enzyme group had a significant 32 percent lower CRP area under the curve in the first week after surgery, along with significantly less self-reported pain (p = 0.034 for the CRP difference).

But the evidence is not one-directional. A 2017 randomized trial took the opposite result. Researchers gave 138 marathon runners either a rutoside-plus-hydrolytic-enzyme supplement (containing bromelain and trypsin) or placebo for one week before and two weeks after a marathon. Despite the large, well-controlled design, the supplement did not significantly reduce the post-race rise in high-sensitivity CRP, interleukin-6, or interleukin-10, and it did not lower the rate of upper respiratory infections afterward. When the same class of enzyme supplement helps in one setting and does nothing in another, it tells you the effect is real but context-dependent, not universal.

More recent work continues that pattern. A 2025 randomized controlled trial of systemic enzyme therapy after orthopedic surgery, which included a trypsin-bromelain-rutoside formulation as a comparator, again reported meaningful reductions in CRP and erythrocyte sedimentation rate against placebo. And a 2024 observational study in people with chronic venous insufficiency found that those taking a supplement containing bromelain (with baicalin and escin) had significantly lower CRP than non-users after 90 days (p = 0.006). The through-line is consistent: bromelain-containing regimens tend to help most where there is an acute inflammatory load to resolve.

The Catch: Most Human Evidence Uses Combinations, Not Bromelain Alone

Here is the honest limitation buried in those impressive surgical numbers. Almost all of the human trials that show bromelain lowering CRP tested it as one ingredient in a blend, typically alongside trypsin and rutoside, sometimes with other flavonoids. That design is useful for products but frustrating for evidence, because it becomes very hard to say how much of the benefit came from the bromelain and how much came from its partners. Rutoside is itself a flavonoid with anti-inflammatory activity, and trypsin is another proteolytic enzyme, so a positive result for the blend is not the same as a positive result for bromelain.

This is a common problem across the enzyme and supplement world, and it is worth being clear-eyed about it. When you read that "bromelain reduces inflammation after surgery," the more accurate statement is usually that "an enzyme combination containing bromelain reduces inflammation after surgery." That does not make bromelain useless, and the preclinical work strongly suggests the enzyme is pulling real weight, but it does mean the isolated-bromelain evidence in humans is thinner than the marketing implies. Bromelain From Pineapple Versus Supplements

A reasonable next question is whether you can just eat pineapple. The answer is mostly no, at least not for a meaningful anti-inflammatory dose. Bromelain is concentrated in the pineapple stem, which is not the part you eat, and the amount in the edible fruit is comparatively small. The doses used in clinical trials, typically in the range of tens to hundreds of milligrams of standardized bromelain per day, are far higher than what a few slices of pineapple would supply. So while pineapple is a genuinely healthy fruit, rich in vitamin C and other nutrients, treating a bowl of it as an anti-inflammatory treatment is a stretch. Two products with the same milligram amount can differ substantially in actual enzyme activity, which complicates any attempt to compare doses across studies or brands. This is one more reason the human dosing picture is less tidy than it looks.

Where Bromelain Looks Most Promising

Recovery from surgery and injury. This is bromelain's strongest evidence base. Multiple randomized trials of enzyme combinations containing bromelain report reduced swelling, pain, and inflammatory markers after procedures ranging from joint replacement to dental and soft-tissue surgery. If bromelain has a legitimate clinical niche, acute post-operative recovery is it.

Osteoarthritis and joint discomfort. Reviews of clinical studies have suggested bromelain's anti-inflammatory and analgesic properties may help with the pain and stiffness of osteoarthritis, though the trials are smaller and more variable than the surgical literature, and again often use combination products.

Sinus and soft-tissue swelling. Bromelain has a long history of use for sinusitis and sports-related bruising and swelling, and some controlled studies support a modest benefit for reducing these acute symptoms. This fits the enzyme's mechanism of speeding the resolution of localized inflammation.

What bromelain has not been shown to do is reliably lower baseline inflammation in otherwise healthy people. The trials that succeed almost always start with an acute inflammatory trigger, such as an operation. There is no good evidence that a healthy person with a normal CRP will drive it lower by adding bromelain, and the marathon trial is a useful caution against assuming that any inflammatory challenge will respond.

Is Bromelain Safe?

Bromelain is generally well tolerated at the doses used in studies, with the most common side effects being mild digestive upset, such as nausea or diarrhea, and occasional allergic reactions, particularly in people allergic to pineapple. Because it is a proteolytic enzyme that can affect clotting, bromelain may increase the risk of bleeding and can interact with blood-thinning medications such as warfarin and antiplatelet drugs. It may also increase the absorption of certain antibiotics, raising their blood levels. People scheduled for surgery are often advised to stop supplements like bromelain beforehand precisely because of the bleeding consideration, which is somewhat ironic given its post-surgical use under medical supervision.
